目的:检测AA组与对照组的免疫球蛋白(IgG、IgA及IgM)含量以及B淋巴细胞亚群计数(CD20+B淋巴细胞,CD19+CD38+B淋巴细胞,CD5+CD19+淋巴细胞亚群),了解两组的体液免疫表达差异。方法:速率散射比浊法检测AA组与对照组的免疫球蛋白(IgG、IgA及IgM)含量,比较免疫球蛋白的表达情况。流式法检测CD20+B淋巴细胞,CD19+CD38+D的活化B淋巴细胞,CD5+CD19+的B淋巴细胞亚群计数。结果:(1)AA组和对照组的各免疫球蛋白浓度无统计学差异,SAA组的免疫球蛋白浓度(IgM,IgG)低于NSAA组,根据患者感染率进行分析,发现SAA组的感染率明显高于NSAA组。(2)AA组的B淋巴细胞亚群中CD20+B淋巴细胞计数明显低于对照组;CD19+CD38+B淋巴细胞,CD19+CD5+B淋巴细胞计数明显高于对照组;CD20+B淋巴细胞计数在SAA组和NSAA组之间无差异,而SAA组的CD19+CD38+B淋巴细胞,CD19+CD5+B淋巴细胞计数明显高于NSAA组;(3)根据AA患者对IST应答率来比较,发现除CD20+B淋巴细胞外,无效组的CD19+CD38+,CD19+CD5+B淋巴细胞亚群计数均高于有效组。结论:(1)免疫球蛋白的浓度与AA的感染相关;(2)AA患者活化的B淋巴细增多提示AA患者存在B淋巴细胞介导的体液免疫紊乱;B淋巴细胞功能紊乱患者对IST疗效差。 |

Objective: To investigate the changes and clinical significance of B-lymphocyte subsets and the immunoglobulin in AA patients. Methods: Use Rate immune scatter turbidimetry to detect the concentration of immunoglobulin (IgG,IgA and IgM)between AA group and control group. Use flow cytometry assay to detect the expression level of CD20+B lymphocyte, activated CD19+CD38+B lymphocyte, B lymphocyte subsetsCD5+CD19+. Results: (1) There is no significant difference in immunoglobulinconcentration between the AA group and the control group. Compared with the AA patients’infection rate, SAA group is higher than that of NSAA group;(2) The expression level of CD20+B lymphocytes in AA group was significantly lower than that of the control group; The expression level of CD19+CD38+, CD19+CD5+B lymphocytes in AA group was significantly higher than that of the control group; And no difference of CD20+B lymphocytes expression level between SAA group and NSAA group,but CD19+CD38+, CD19+CD5+B lymphocytes expression levels in NSAA were lower that of SAA group ;(3) Compared with the AA patients’ respondingto IST, the effective patients have a significantly lower expression level of CD19+CD38+, CD19+CD5+B lymphocytes than that of transfusion dependent patients , the differences were statistically significant. Conclusions:(1) The infection rate of AA may be associated with immunoglobulin;(2) The increased activated B cells and the disorder of humoral immune;The conventional IST treatment was less effective in B lymphocyte dysfunction patients. |
