| 【】 目的 观察帕利哌酮与阿立哌唑对首发精神分裂症患者的糖脂代谢、胰岛素、瘦素水平、体质量指数的影响。方法 将70例首发精神分裂症患者随机分为研究组(帕利哌酮组,35例)和对照组(阿立哌唑组,35例),观察8周。以阳性和阴性症状量表(PANSS)评估疗效,治疗时出现的症状量表(TESS)评估不良反应,同时于治疗前和治疗后第4、8周末检测两组患者的血糖、血脂、胰岛素、瘦素及体质量、身高,并计算体质量指数。结果 基线时两组各代谢指标及PANSS评分无统计学差异(P>0.05)。至研究终点,两组甘油三酯、瘦素水平均较治疗前有所升高(P﹤0.05),组间代谢指标变化无差异(P>0.05)。治疗后两组PANSS总分有明显下降(P﹤0.01),有效率相似(65.7%vs 62.9%,P>0.05)；两组均未出现严重不良反应。结论 帕利哌酮对精神分裂症患者代谢相关指标影响与阿立哌唑相似。 |

| 【】 Objective To observe the changes of metabolic indicators in the treatment of first-episode schizophrenia with paliperidone or aripiprazole. Method 70 schizophrenia inpatients were randomly assigned to study group (paliperidone,n=35) and control group (aripiprazole,n=35). The plasma levels of leptin, the fasting insulin(FINS), fasting blood glucose(FBG),blood lipids (triglyceride , total cholesterol, low density lipoprotein cholesterol and high density lipoprotein cholesterol TG,TC,LDL and HDL) ,body mass index(BMI)were measured before and after 4 weeks, 8 weeks. The clinical efficacy was evaluated with the Positive and Negative Syndrome Scale (PANSS). Side effects were done with the Treatment Emergent Symptom Scale (TESS). Results There were no significant differences between paliperidone and aripiprazole groups on the levels of metabolic indicators and PANSS total scores before treatment (P>0.05). The TG,leptin levels of two groups were elevated after treatment ( P＜0.05). There was no difference the changes of metabolic indicators between two groups. PANSS total scores decreased significantly after treatment( P＜0.01) ,and the effective rate was similar (65.7%vs 62.9%, P>0.05 )after 8 weeks of treatment. Conclusion Paliperidone has fewer effects on the metabolic indicators in first-episode schizophrenia patients as well as aripiprazole. |
