| 目的 探讨B超和CT在非典型急性阑尾炎诊断中的价值。方法 回顾性分析2011年1月至2015年6月于我院收治的156例疑似急性阑尾炎的临床资料,其中141例非典型急性阑尾炎,15例急性阑尾炎阴性。比较B超和CT检查诊断非典型急性阑尾炎的差异。结果 CT诊断非典型急性阑尾炎的敏感性和准确性均高于B超,差异有统计学意义(均P＜0.05)。两种检查方法诊断非典型急性阑尾炎的特异性、阳性预测值和阴性预测值比较,差异无统计学意义(均P>0.05)。单纯性阑尾炎患者行CT检查的诊断敏感性高于B超检查,且差异有统计学意义(P＜0.05)。但是对于化脓性、坏疽性或穿孔性阑尾炎及阑尾周围脓肿患者,两种检查方法的诊断敏感性比较,差异无统计学意义(均P>0.05)。结论 对于非典型急性阑尾炎患者,CT检查优于B超,尤其是单纯性阑尾炎。 |

| Objective To evaluate the diagnostic performance of type-B ultrasound and computed tomography (CT) for atypical appendicitis. Methods The clinical data of 156 patients with suspected acute appendicitis from Jan. 2011 to Jun. 2015 were reviewed. Among them, 141 cases were diagnosed as atypical appendicitis and the other 15 cases were diagnosed as other diseases. The differences in the diagnostic performance between type-B ultrasound and CT for atypical appendicitis were compared. Results The sensitivity and accuracy of type-B ultrasound were higher than those of CT (All P＜0.05 ), but the difference in the specificity, negative and positive predictive value between the diagnostic methods were not statistically significant (All P>0.05). The sensitivity of type-B ultrasound for simple appendicitis was higher than that of CT (P＜0.05), but the difference for the other types of appendicitis were not statistical (All P>0.05). Conclusions CT shows a better diagnostic performance than that of type-B ultrasound for the diagnose of atypical appendicitis, especially for simple appendicitis. |
