【】目的通过CT测量主肺动脉/降主动脉内径比值,分析其与慢性阻塞性肺疾病(COPD)相关肺心病患者肺动脉高压及肺功能的关系。方法 82例确诊COPD合并肺源性心脏病患者接受胸部CT检查,选择同一层面测量主肺动脉及降主动脉内径,计算肺动脉/降主动脉内径比值(rPDA),同时采用超声心动图测量其肺动脉压力(MPAP值),分析rPDA与肺动脉压力及肺功能相关性,并与30例健康体检者进行比较。结果患者组 rPDA值及MPAP值均较对照组明显升高(P＜0.05),肺功能指标(FEV1﹑FVC﹑FEV1占预计值%﹑FEV1/FVC%)均较对照组明显降低(P＜0.01,P＜0.05)；相关性分析表明, rPDA值与FEV1占预计值及FEV1/FVC均呈明显负相关(r=-0.524、-0.671,P＜0.01),与MPAP值呈明显正相关(r=-0.723,P＜0.01)。结论 CT测量rPDA值是诊断COPD相关性肺心病患者肺动脉高压可靠而敏感的指标,且可一定程度反应其肺功能变化,值得临床推广应用。

【】　Objective To investigate the relationship between the ratio of main pulmonary artery diameter/descending aorta (rPDA)as measured on computed tomography(CT) and the lung function of COPD patients complicated with pulmonary heart disease. Methods 82 COPD patients complicated with pulmonary heart disease were examined by chest CT,the diameter of main pulmonary artery and descending aorta were measured at same level.The mean pulmonary artery pressure(mPAP) were measured by ultrasound cardiogram.the relationship between rPDA and mPAP 、lung function.,while the 30 healthy examiners were compiled into the control group.Results The ratio of rPDA and mPAP were increased significantly than control group (P＜0.05),while the index of lung function(FEV₁﹑FVC﹑FEV₁/FVC% and FEV₁ predicted )were decreased significantly than healthy control group(P＜0.01,P＜0.05). The ratio of rPDA was positively correlated with FEV₁/FVC% and FEV₁ predicted(r=-0.524、-0.671,P＜0.01)；but the ratio of rPDA was negaitive correlated with MPAP(r=-0.723,P＜0.01).Conclusions The ratio of rPDA as measured by CT is reliable and sensitive indicators to diagnose pulmonary heart disease complicated with pulmonary artery hypertension,which is realated with lung function.
