      目的 探讨我院血液科患者医院感染的病原菌分布及其对常用抗菌药物的耐药性,为临床合理应用抗菌药物提供参考。方法 选取2013年1月至2014年6月血液科送检的标本397例,通过进行菌株培养、鉴定和药敏试验,描述其病原菌构成和耐药性。结果 血液科医院感染发病率为13.6FfFf,发病部位以上呼吸道(占47.6FfFf)为主。革兰阴性菌241株,占60.7FfFf;革兰阳性菌115株,占29.0FfFf;真菌41株,占10.3FfFf。主要革兰阴性菌对头孢哌酮/舒巴坦、美罗培南、亚胺培南耐药率较低,对氨苄西林、头孢噻肟的耐药率较高;主要革兰阳性菌对万古霉素、呋喃妥因、替考拉林的耐药率较低,对青霉素G、红霉素的耐药率很高。结论 血液科医院感染主要以革兰阴性菌为主,尤其以大肠埃希菌、铜绿假单胞菌和肺炎克雷伯菌常见,临床应按照病原菌药敏结果,合理应用抗菌药物,减少耐药性。

      Objective To investigate the distribution of pathogens in hematology department and its drug resistance to commonly used antimicrobial agents as to provide the basis for clinical application of antibacterial drugs. Methods 397 specimens of pathogens in hematology department from 2013 January to 2014 June were collected, through strain culture, identification and drug sensitivity test, we described the composition of pathogenic bacteria and drug resistance. Results The incidence of nosocomial infection in the hematology department was 13.6FfFf, mainly occurred at the upper respiratory tract (47.6FfFf). 241 were strains of gram negative bacteria, accounting for 60.7FfFf; 115 were strains of gram positive bacteria, accounting for 29FfFf; 41 were strains of fungi, accounting for 10.3FfFf. Mainly Gram-negative bacteria to cefoperazone/sulbactam, meropenem, imipenem were lower, higher resistance rates to ampicillin, cefotaxime; mainly Gram-positive bacteria to vancomycin, nitrofurantoin, teicoplanin were lower, higher resistance rates to penicillin G, erythromycin. Conclusion Hospital infection in hematology department are mainly Gram-negative bacteria, especially Escherichia coli, Pseudomonas aeruginosa and Klebsiella pneumoniae, it should be rational use of antimicrobial drugs according to the pathogens drug sensitive results to reducing resistance.
