| 【摘要】目的 探讨单唾液酸四已糖神节经苷酯(GM-1)联合大剂量丙种球蛋白(IVIG)治疗儿童重型病毒性脑炎(SVE)的临床效果。方法 将73例患儿随机分为3组，对照组25例，治疗A组24 例和治疗B组24例。对照组给予常规治疗，治疗A组在常规治疗基础上给予大剂量IVIG治疗，治疗B组在治疗A组基础上加用GM-1治疗。比较3组患儿临床症状消失时间、脑电图恢复正常时间、治疗前后血清神经元特异性烯醇化酶（NSE）及白细胞介素-6(IL-6)变化。结果 治疗A组、B组意识转清、惊厥控制、热退、头痛呕吐消失、肢体活动恢复、脑电图恢复正常时间及NSE、IL-6水平明显少于对照组（P＜0.05）；治疗A组、B组总有效率及显效率均明显高于对照组（P＜0.05）；治疗B组肢体活动恢复、脑电图恢复正常时间及NSE、IL-6水平明显低于治疗A组（P＜0.05），治疗B组总有效率及显效率均明显高于治疗A组，差异均有统计学意义。两组在意识转清、惊厥控制、热退、头痛呕吐消失时间方面均无明显差异(P＞0.05)；三组均无严重不良反应发生。结论 GM-1联合大剂量IVIG治疗小儿SVE有良好的临床疗效，值得临床推广应用。 |

| Abstract: [Objective]To explore effect of ganglioside combined with high-dose immunoglobulin in the treatment of children with severe viral encephalitis(SVE).[Method] 73 cases of severe viral encephalitis children were randomly divided into the control group （25 cases）,the treatment group A （24 cases）and the treatment group B（24 cases）.The control group was treated with conventional treatment.The treatment group A was administered the routine treatment combined with high doses of immunoglobulin.The treatment group B was administered the ganglioside based on the treatment group A.The clinic effects, electroencephalogram (EEG)recovery time，the level of NSE and Interleukin-6 before and after treatment were compared among the three groups.[Result] The recovery of consciousness,time to control of convulsions,abatenient offever,disappearance of headache and vomiting,Physical activity recovery,EEG recovery,the level of NSE and Interleukin-6 in the treatment group A and B were shorten than that of in the control group (P<0.05)．The total effective rate and significant effective rate in the treatment group A and B was significantly higher than that of in the control group. Physical activity recovery,EEG recovery ,the level of NSE and Interleukin-6 in the treatment group B were shorten than that of in the treatment group A(P<0.05).The total effective rate and significant effective rate in the treatment group B was significantly higher than that of in the treatment group A. All the differences were statistically significant.There were no significant difference in the recovery of consciousness,time to control of convulsions,abatenient offever,disappearance of headache and vomiting between the treatment group A and treatment group B (P＞0.05).Three groups were no serious adverse events. [Conclusion] ganglioside combined with immunoglobulin in the treatment of children with severe viral encephalitis is efficacious in the treatment of severe viral encephalitis．It is worth popularzing clinically. |
