      目的:探讨中药对2型糖尿病肾病的治疗作用,同时结合CX3CR1-V249I基因多态性进行中药疗效分析。方法: 108例患者被随机分为对照组(西医治疗)和中药组(西医治疗联合中药)。全部患者在治疗开始前均接受CX3CR1-V249I基因多态性的检测。分别于治疗前、后抽血及收集尿液检测糖化血红蛋白、尿白蛋白、尿α1微球蛋白(α1MU)、血β2微球蛋白(β2MU)、FKN、NF-κ b、IL-6、TNF-α等指标。结果:所有患者中CX3CR1-V249I野生型87例,突变型21例。治疗后各组不同基因型患者尿中白蛋白、尿α1MU、血β2MU较治疗前均有明显下降(p<0.05)。但中药组中突变型明显优于对照组(p<0.05)。中药组治疗后血清中NF- κB、FKN、IL-6、TNF-α含量治疗后较治疗前明显减少(p<0.05),在突变型患者中这种下降更明显,差异有统计学意义(p<0.05)。结论:参芪地黄汤能有效减少2型糖尿病肾病患者尿白蛋白、尿α1微球蛋白(α1MU)、血β2微球蛋白(β2MU)、FKN、NF-κ b、IL-6、TNF-α含量,保护肾脏组织,这一作用在CX3CR1-V249I突变型患者中更加明显。

      Objective: This study aimed to determine the function of herbs in Treatment of type 2 diabetic nephropathy and analyze the efficacy of the herbs based on CX3CR1-V249I gene polymorphism. Methods:A total of 108 patients were randomly divided into the control (western medicine therapy alone) and herb (western medicine therapy combined with herbs)groups. Such indexes as blood HbA1c, blood β2MU、FKN、NF-κ b、IL-6、 TNF-α 、urinary albumin and urinary α1MU were measured before and after treating. Results: A total of 87 patients had VV wild genotype,and another 21 patients had non-wild genotype. The level of urinary albumin 、urinary α1MU and blood β2MU were markly lowered in both groups (P<0.05). In the herb group, the levels of NF- κB、FKN、IL-6 and TNF-α were decreased after TCM administration (p<0.05),especially in patients with non-wild genotype. Conclusion: Shengqi Dihuang Decoction could decrease the levels of blood β2MU、FKN、NF-κ b、IL-6、 TNF-α 、urinary albumin and urinary α1MU and attenuate inflammation in kidney,especially in patients with non-wild genotype.
