| 【】 目的 探讨高血压社区门诊管理中影响高血压患者遵医嘱服药的影响因素,寻求针对提高社区高血压患者的遵医行为提供参考。 方法 随机抽取在乌镇镇社区服务中心管理一年以上的5692例中的359例高血压患者作为研究对象。由社区医生按照统一要求对研究对象进行问卷调查,内容包括性别、年龄、职业、受教育程度、收入水平、管理形式、高血压认知程度等。 结果 对359例社区高血压患者进行问卷调查,有效问卷345份,有效率96.1％,其中遵医嘱服药≥12个月281例(A组) 、遵医嘱服药＜12个月25例(B组)、不能遵医嘱服药39例(C组)。(1)3组在文化程度、高血压认知度、管理方式上差异有统计学意义(P＜0.05)；其中B组患者的文化程度、高血压认知度与C组比较；A组患者的高血压认知度与B组比较；A、B组患者的管理形式与C组比较,差异有统计学意义(P＜0.05)。(2)进行多分类Logistic回归分析,在A组中,相对于一般管理,用强化管理并有家庭成员参与这种方式管理的高血压患者遵医嘱服药坚持12个月以上,优势比(OR)＞2 (P＜0.01)。结论 社区高血压患者遵医嘱服药与社区医生对患者的管理方式有关,所以加强对社区高血压患者的管理是改变其遵医行为的关键。 |

| Objective To investigate the factors affecting the management of hypertension in patients with community clinics prescribed hypertension medication , seek to provide a reference for improving the community"s compliance behavior in hypertensive patients.Method Randomly in Wuzhen Town Community Centre Management 5692 cases a year or more in 359 hypertensive patients as research subjects .SBy community physicians in accordance with the standard requirements of the study questionnaire , including gender, age , occupation, education level , income level, forms of management , hypertension awareness and so on.S Results 359 cases of patients with hypertension community questionnaire,345 valid questionnaires , efficiency of 96.1% , which was prescribed medication ≥ 12 months, 281 cases (group A) , prescribed medication < 12 months, 25 patients ( group B)Snot prescribed medication 39 cases ( group C).S( 1 ) 3 groups on education, hypertension awareness, management was statistically significant (P <0.05) differences ; one of the more educated group B patients , hypertension awareness and group C; A group of patientsSawareness of hypertension compared with group B; A, group B and group management of patients in the form of C , the difference was statistically significant (P <0.05).S( 2 ) multi- LogisticSregression analysis , in group A , with respect to general management , with a strengthened management and family members to participate in the management of hypertension in this manner prescribed medication persist more than 12 months , the odds ratio (OR)S> 2 (P <0.01).SConclusions Patients with community physicians prescribed medication for management of hypertension in patients with community- related , so strengthen the management of patients with hypertension community is the key to its compliance behavior change .#$NLKeywords Hypertension ; Prescribed medication; Community health services ; influencing factors |
