| 目的：探讨采用双缝合锚治疗锁骨远端Neer II b型骨折的临床疗效。方法：回顾性分析2010年8月至2013年8月，采用双缝合锚治疗18例Neer IIb型锁骨远端骨折的资料，男性11例，女性7例；年龄21 ～59岁，平均41.2 岁；右侧8例，左侧10例；损伤至手术时间为24h-7d，平均50h。所有患者均采用手术治疗，术中骨折复位满意后将两枚缝合锚固定于喙突基底部，予骨折近端钻两个1.6 mm的骨孔，将锚钉尾线穿过骨孔，打结固定。术后采用Constant-Murley评分系统评价术后肩关节功能，疼痛采用视觉模拟法(VAS)进行评定。结果：所有患者均获得随访，随访时间为12-18个月，平均16.5个月。X线片显示锁骨骨折愈合，愈合时间为8-15周，平均愈合时间11.5周。18例患者的Constant-Murley评分为82-100分，平均94.5分；VAS评分平均为1.5分(0-4分)，结论：采用双缝合锚治疗锁骨远端Neer IIb型骨折时可以稳定锁骨远端，避免经肩关节固定锁骨远端的并发症，无需行内固定取出术。 |

| Objective To explore the clinical efficacy of using two suture anchors in the treament of type IIb fractures of the distal clavicle. Method Between Aug 2010 and Aug 2013, 18 patients were treated using two suture anchors for type IIb fractures of the distal clavicle. There were 11 men and 7 women, with the mean age of 41.2 years (range, 21-59 years). The right shoulder was involved in 8 patients and the left in 11 patients. The interval time between injuries and operation was 24h-7d (average, 50 h). After the fracture reduction, two suture anchors was inserted into the base of the coracoid process. Then two 1.6 mm clavicular holes were drilled. The suture strands were passed trough the holes, and then were tied. The shoulder function was assessed with Constant-Murley scoring system, and the pain was evaluated according to visual analog scale(VAS)． Results All patients were followed up, and the duration of follow-up was 12-18 months (mean, 16.5 months). The radiological examination showed that fracture union was eventually achieved in all patients. The constant-Murley scores of all patients were 82-100 (mean, 94.5). Their mean VAS score was 1.5(0 to 4) Conclusions The suture anchor fixation in the treatment of type IIb fractures of the distal clavicle is a reliable and simple technique． |
