| 目的 观察利奈唑胺治疗耐多药肺结核合并结核性脑膜炎的临床疗效、长疗程用药的安全性及不良反应。方法 对1例耐多药肺结核合并结核性脑膜炎患者，原耐多药化疗方案治疗过程中病情加重、恶化，加用利奈唑胺后明显改善的临床资料进行回顾性分析，观察患者临床症状、细菌学改善、影像学表现和不良反应，并结合文献进行讨论。结果 患者接受利奈唑胺600mg，每日2次联合原方案抗结核治疗，病情迅速改善，症状明显缓解，利奈唑胺共计用药4月，肺部阴影吸收，痰涂片找抗酸杆菌阴转，随访16月，病情无反复，且未见明显药物不良反应。结论 耐多药结核病尤其合并结核性脑膜炎患者化疗过程出现病情反复，加用利奈唑胺对提高疗效、改善预后有一定意义。 |

| OBJECTIVE To investigate the efficacy and safety of linezolid in the treatment of against tuberculous meningitis infection. METHODS Case report and systematic review about linezolid for the treatment of patient with XDR tuberculous and meningitis. RESULTS A patient with MDR tuberculous and meningitis were treated by conventional anti-tuberculous treatment and use of linezolid, 600mg intravenous drip bid. The clinical symptoms, including headache, high fever, and unconsciousness etc, were all relieved. Abnormality of cerebrospinal fluid was improved. And sputum-smear conversion was got. During the period of the 4 months" treatment and 10 months" follow-up, no severe adverse reaction be found. CONCLUSION Linezolid could be used effectively and safely in the treatment of patients with MDR tuberculous and meningitis. |
