      目的:探讨三种不同血透方式对老年尿毒症患者钙磷代谢及甲状旁腺激素的影响,阐明三种血透方式在老年尿毒症患者血液透析中的优势。方法:选取2011年到2013年来我院进行血液透析治疗的老年患者90例,通过随机分组的方式将其分为常规组30例、滤过组30例和高通量组30例。其中常规组患者给予常规血液透析,滤过组患者给予血液透析滤过,高通量组患者给予高通量血液透析,6个月后采用全自动生化仪检测患者血液中钙磷以及甲状旁腺激素的水平。结果:经过6个月的治疗后高通量组患者的钙含量大于常规组和滤过组(P<0.05),而滤过组患者的钙含量大于常规组(P<0.05);高通量组患者的磷含量小于常规组和滤过组(P<0.05),滤过组患者的磷含量小于常规组(P<0.05);常规组患者的钙磷乘积大于滤过组和高通量组,滤过组的钙磷乘积大于高通量组(P<0.05);滤过组患者的甲状旁腺激素量小于高通量组和常规组,高通量组患者的甲状旁腺激素量小于常规组(P<0.05)。结论:高通量血液透析对低钙血症、高磷血症的治疗优于普通血液透析及血液透析滤过;而血液透析滤过对中大分子的清除明显优于普通血液透析及高通血液透析。

      Objectives:To analyze the influence of different hemodialysis methods on calcium,phosphorus metabolism and parathyroid hormone in elderly patients with uremia to elucidate the advantage of three hemodialysis in uremic. Methods:Divided 90 cases of elderly patients who did blood dialysis treatment from 2011 to 2013 in our hospital into conventional group(n=30), filtration group(n=30), high throughput group(n=30) by random. Conventional group was treated with conventional hemodialysis, filtration group was treated with hemodialysis filtration and high throughput group was treated with high volume hemofiltration. After 6 months of treatment, blood calcium and phosphorus and parathyroid hormone levels were detected by automatic biochemical analyzer. Results: After 6 months of treatment ,the calcium content of high throughput group were larger than conventional group and filtration group (P < 0.05), while the content of calcium in filtration group were higher than those of conventional group (P < 0.05), phosphorus content in high throughput group were less than conventional group, filtration group (P < 0.05), and phosphorus content in filtration group were lower than conventional group (P < 0.05),calcium-phosphorus product of conventional group was greater than that of filtration group and high throughput group, And the calcium-phosphorus product of filtration group was greater than that of high throughput group (p<0.05),parathormone quantity of filtration group was lesser than that of conventional group and high throughput group, parathormone quantity of high throughput group was lesser than conventional group (p<0.05).Conclusions: High volume hemofiltration is better than the conventional hemodialysis and hemodialysis filtration for the treatment of hypocalcemia and hyperphosphatemia, besides hemodialysis filtration is obviously better than conventional hemodialysis and high volume hemofiltration in the clearance of middle molecule and macromolecules.
