姜黄素治疗睾丸缺血再灌注损伤的实验研究

      目的 睾丸扭转复位后的损伤是一个缺血再灌注损伤。睾丸缺血再灌注后大量活性氧的产生是导致睾丸损伤的主要原因。本研究评价了抗氧化剂姜黄素对睾丸缺血再灌注损伤的影响。 方法 60只成年雄性SD大鼠被分为3组。对照组经历了左侧睾丸假手术。在缺血再灌注组,左侧睾丸扭转720°,2小时后复位。治疗组大鼠接受了与缺血再灌注组一样的外科手术,但在睾丸复位时,给予姜黄素静脉注射。复位后4小时,每组一半的大鼠行睾丸切除术,测定黄嘌呤氧化酶活性(该酶催化活性氧产生)、丙二醛水平(活性氧标志)和血红素加氧酶-1的蛋白表达水平(该酶催化抗氧化剂产生)。复位后3个月,剩余的大鼠行睾丸切除术,分析睾丸生精功能。结果 单侧睾丸扭转复位引起同侧睾丸黄嘌呤氧化酶活性、丙二醛水平和血红素加氧酶-1的蛋白表达水平显著升高,睾丸生精功能明显下降。用姜黄素治疗后,同侧睾丸的黄嘌呤氧化酶活性和丙二醛水平显著下降,血红素加氧酶-1的蛋白表达水平和睾丸生精功能明显提高。 结论 姜黄素通过抑制黄嘌呤氧化酶活性、提高血红素加氧酶-1的蛋白表达水平来减少活性氧含量,从而保护睾丸的生精功能。

      Objective The pathophysiology of testicular torsion-detorsion is ischemia-reperfusion injury of testis. The ischemia-reperfusion injury is associated with overgeneration of reactive oxygen species (ROS). This study evaluated effect of curcumin, a potent antioxidant, on testicular ischemia-reperfusion injury. Methods Sixty adult male Sprague-Dawley rats were randomly divided into three groups. Rats in control group underwent a sham operation of left testis. In ischemia-reperfusion group, left testis was rotated 720°for 2 hours. Rats in treatment group received same surgical procedure as ischemia-reperfusion group, but curcumin was administered intravenously at repair of testicular torsion. Orchiectomy was performed on half of rats in each group at 4 hours after repair of torsion for measurement of activity of xanthine oxidase, which catalyzes production of ROS, malondialdehyde level(an indicator of ROS content),and protein expression level of heme oxygenase-1, which catalyzes antioxidant generation. Orchiectomy was performed on remaining rats at 3 months after repair of torsion, to allow analysis of testicular spermatogenesis. Results Unilateral testicular torsion-detorsion caused significant increases in xanthine oxidase activity, malondialdehyde level, and heme oxygenase-1 protein expression level, and caused a significant decrease in testicular spermatogenesis in ipsilateral testes. Rats treated with curcumin had significant decreases in xanthine oxidase activity and malondialdehyde level, and had significant increases in heme oxygenase-1 protein expression level and testicular spermatogenesis in ipsilateral testes. Conclusion Curcumin protects testicular spermatogenesis by reducing ROS content by inhibiting xanthine oxidase activity and improving heme oxygenase-1 protein expression level.
