      目的:观察膝关节控制训练对脑卒中后痉挛性偏瘫最大等长收缩的表面肌电图特征,为科学性的治疗及评价痉挛性偏瘫提供实验依据。方法: 选取脑卒中后痉挛性偏瘫患者80例,将其随机分为对照组及治疗组各40例,分别采用传统康复疗法和膝关节控制训练疗法共进行治疗4周。治疗前后采用临床痉挛指数评定及表面电极引导和记录肌电信号并进行线性时、频分析。结果: 治疗组治疗后的临床痉孪指数显著降低, 而对照组无显著变化, 两组有显著差异(P<0.05)。治疗组患侧膝屈曲最大等长收缩时,膝屈曲的协同收缩率降低, 与治疗前相比差异均有显著意义(P<0.01) , 且各项指标均与对照组有显著性差异(P<0.01)。结论: 膝关节控制强化训练对提高脑中风后痉挛性偏瘫患者患侧膝关节的控制能力,减轻患侧下肢的肌张力的效果明显好于传统康复疗法疗法。
英文摘要:
      Objective: To observe the effects of surface electromyography signal of Control training of knee joint on maximum isometric voluntary contraction of spastic hemiplegia patients. Methods: 80 patients with who suffers spastic hemiplegia were divided into two groups. There were 40 patients in observation group which were treated by the traditional rehabilitation training and Control training of knee joint, and 40 patients in control group which were treated only by the traditional rehabilitation therapy training. The electromyography signals were collected by surface electro de and then processed by linear time-and frequency-domain method. And then the Clinic Spastcity Indexwas analyzed with factors statistics.Results: four weeks after treatment, he clinical spasm index decreased significantly, and observation group are more apparently than control group(P<0.05). co-contraction ratio were decreased with before treatment during knee maximum isometric voluntary contraction (P<0.01), and observation group are more apparently than control group(P<0.01). Conclusion: Strengthened control training of knee joint can improve the control ability of spastic hemiplegia patients of knee joint, and effectively relieve muscle tension in the lower limb of the effects are better than traditional rehabilitation therapy.
