      目的:探讨联合检测多种生化指标与肿瘤指标诊断HCC的临床价值。方法:回顾性分析88例HCC临床资料,并以肝硬化及健康体检者分别作为试验对照组。记录所有受检者生化指标TBil、Alb、ALP、ALT、GGT与肿瘤指标CA125、CA199、CEA及AFP。两独立样本t检验比较病例组与肝硬化对照组生化指标及肿瘤指标组间差异;计算生化指标与肿瘤指标单独与联合检测诊断HCC的灵敏度及特异性。结果:两独立样本t检验结果显示:HCC病例组与肝硬化组ALT、ALP与GGT值组间差异具有统计学意义(t=2.443,1.986,2.321,P均<0.05);CA125、CA199、AFP值组间差异具有统计学意义(t=1.994,14.676,21.422,P均<0.05)。生化指标ALP、ALT、GGT诊断HCC的灵敏度分别为56.8%、40.9%和72.3%,特异性分别为80.0%、52.5%和72.5%。肿瘤指标CA125、CA199、AFP诊断HCC灵敏度分别为50.0%、48.9%和73.9%,特异性分别为82.5%、80.0%和85.0%。以GGT与三项肿瘤指标CA125、CA199、AFP联合检测灵敏度最高,为88.6%,而ALT和GGT与三项肿瘤指标联合检测特异性一致,均为87.5%。结论:生化指标与肿瘤指标联合检测可提高诊断HCC的灵敏度及特异性,可为临床工作的开展提供一定的数据支持。

      Objective To study the diagnostic value of combined detection of biochemical markers and tumor markers in patients with HCC. Methods 88 patients with HCC, 80 patients with cirrhosis and 40 healthy controls were included in this study. All biochemical markers and tumor markers were recorded. SPSS11.5 was used for statistic analysis. Results There were significant differences in ALT, ALP and GGT between the groups of HCC and cirrhosis (all P<0.05), the sensitivity were 56.8%, 40.9% and 72.3% respectively, the specificity were 80.0%, 52.5% and 72.5% respectively. There were significant differences in CA125, CA199 and AFP between the groups of HCC and cirrhosis (all P<0.05), the sensitivity were 50.0%, 48.9% and 73.9% respectively, the specificity were 82.5%, 80.0% and85.0% respectively. The maximum sensitivity was 88.6% with combined detection of GGT, CA125, CA199 and AFP, the maximum specificity was 87.5% with combined detection of ALT or GGT with CA125, CA199 and AFP. Conclusions Combined detection of biochemical markers and tumor markers in patients with HCC had important clinical value in improving diagnostic sensitivity and specificity, which could provide support for clinical work.
