      目的:观察参附强心合剂对心衰大鼠心功能及水通道蛋白2 (Aquaporin 2,AQP2)水平的影响,探讨参附强心合剂抗心衰水潴留的作用机制。方法:用腹主动脉缩窄法制作心衰大鼠模型,挑选造模成功大鼠60只随机分为五组:参附强心合剂大、中、小剂量组,氯沙坦组,模型组。前三组分别按成人用药剂量20、10、5倍剂量的参附强心合剂灌胃(即:参附强心合剂14g.kg-1.d-1、7 g.kg-1.d-1、3.5 g.kg-1.d-1);氯沙坦组以成人剂量10倍的氯沙坦灌胃(即:氯沙坦10mg.kg-1.d-1);模型对照组以蒸馏水灌胃。另假手术组大鼠以蒸馏水灌胃作为空白对照组。给药4周后,行心超检查测定心功能指标,用ELISA法检测大鼠尿AQP-2的浓度,western blot检测大鼠肾脏AQP-2蛋白表达水平。结果:参附强心合剂可改善心衰大鼠心功能,其各剂量组AQP-2的浓度明显下降,差异具有统计学意义(P<0.01或P<0.05);随参附强心合剂剂量的增加, AQP-2浓度逐渐下降,EF值逐渐升高;EF值与AQP-2之间有良好的线性负性相关关系。结论:参附强心合剂可有效改善心功能,改善AQP-2浓度,且呈一定的剂量依赖性;参附强心合剂抗心衰水潴留的作用机制与降低心衰大鼠AQP-2的浓度有关。
英文摘要:
      Objective To determinate the effect of Shenfu qiangxin(SFQX) decoction in chronic heart failure(CHF) rats model. To observe the influence of SFQX decoction on the cardiac function and urinary Aquaporin 2(AQP-2) in CHF rats, in order to explore mechanism of SFQX decoction on water retention in CHF rats model.Methods Using male adult SD rats made CHF models by coarctation of abdominal aorta (CAA). 60 successfully established CHF rats were randomly divided into 5 groups: large-dose group, medium-dose group, low-dose group, Losarton group, and model group. Large-dose group (14 g.kg-1.d-1, SFQX decoction pretreatment), medium-dose group (7 g.kg-1.d-1, SFQX decoction pretreatment), low-dose group (3.5 g.kg-1.d-1, SFQX decoction pretreatment), Losarton group (10mg.kg-1.d-1), sham group and model group(2ml/d sterile purified water pretreatment). At the end of 4 weeks, the cardiac function was measured by ultrasonic echocardiogram, the urinary AQP-2 was detected by ELISA and western blot method.Results Comparing with model group, the cardiac function were improved. The concentration of AQP-2 in each dose SFQX decoction were reduced(p<0.01). The concentration of AQP-2 reduced correspondingly with the increase of the dose of SFQX decoction,while ejection fraction(EF) reducing. There was a remarkable negative correlation between EF and the concentration of urinary AQP-2. Conclusion SFQX decoction can improve the cardiac function,and reduce the concentration of AQP-2 in CHF rats model, which is in a dose-dependent manner. The mechanism of SFQX decoction on water retention in CHF rats may be related to reduced concentration of AQP-2.
