| 目的 探讨高频超声对结节性甲状腺肿背景下甲状腺癌的诊断价值。方法 回顾性分析经病理诊断为结节性甲状腺肿的107例患者的甲状腺结节高频超声声像图表现,并比较分析良恶性结节的声像图特点。结果 107例患者237个结节中有46例73个结节为甲状腺癌，恶性结节的声像图表现为以下其中的一项或者几项：纵径≥横径（形态不规则）、 边界不规整、 内部有微小钙化（≤2mm）、 周围组织浸润、 颈部淋巴结肿大等，多项指标同时评价结节性甲状腺肿合并恶性结节的特异性，准确性，阳性预测值均高于单项指标。结论 高频超声检查对发现甲状腺内的恶性结节具有重要的临床意义, 综合分析甲状腺结节的声像图征象, 可提高结节性甲状腺肿合并甲状腺癌的检出率。 |

| Objective: Discussed under the background of high frequency ultrasonography in nodular goiter to assess the diagnostic value of thyroid carcinoma.．Methods：By pathological diagnosis of nodular goiter were retrospectively analyzed 107 cases of thyroid nodules in patients with high frequency ultrasonic ultrasonographic performance, and comparative analysis of the ultrasonographic characteristics of benign and malignant nodules ．Results：237 nodules in 107 patients with thyroid cancer, 46 cases of 73 nodule in the malignant nodules ultrasonographic performance for the following one or several: longitudinal diameter or greater diameter (irregularly), irregular, boundary internal tiny calcification (2 mm or less), the surrounding tissue infiltration, cervical lymph node enlargement and so on, a number of indicators and to evaluate the specificity of nodular goiter with malignant nodules, accuracy, positive predictive value are higher than that of single index. Conclusion：High frequency ultrasound to find the thyroid malignant nodules has important clinical significance,Comprehensive analysis of thyroid nodules ultrasonographic signs, can improve the detection rate of nodular goiter with thyroid carcinoma. |
