| 【】目的：分析宿主免疫正常的成人肺隐球菌病(PC)的肺部CT表现,提高对本病的认识及诊断水平。方法：回顾分析21例经病理或细菌学证实的PC患者的肺部CT图像及临床资料。结果：(1)肿块或结节型：14例,其中孤立结节3例,多发结节8例,肿块伴结节3例。以两下肺、外周分布为主。多数伴有晕征,部分肿块有空洞形成。(2)斑片型：5例,多发斑片影4例,大片实变影1例。其中实变影有支气管通气征,边界较清。多发病灶双肺散在分布,伴有磨玻璃影。(3)混合型,1例,表现为结节影、斑片影混合存在。(4)弥漫型,1例,表现为小结节影,以胸膜下为主。 结论：肺隐球菌病CT表现多样,病灶以下肺及外周分布为主,多发结节或肿块多见,晕征、空洞、磨玻璃影、支气管通气征有一定提示意义,具有一定的特征性。确诊尚依赖病原学及病理检查。 |

| 【】 Objective：To analyze the CT findings of pulmonary cryptococcosis (PC) in adults with normal immune function for a better understanding and diagnosis of the disease． Methods： CT images and clinical records of 21 patients with histological or clinical confirmed PC were retrospectively reviewed. Result：(1) Mass or nodule type(14 cases): the isolated nodes (3 cases), multiple nodules (8 cases), tumor and nodules (3 cases). The lesions were most distributed in lower lungs of both sides and periphery. Most had halo sign, and some tumors had holes. (2) The patch type (5 cases): multiple patch shadow (4 cases), large consolidation shadow (1 case). Consolidation shadows had air bronchogram and clear boundary. Multiple lesions were scattered both lungs, with ground-glass shadow. (3) Mixed type (1 case): characterized by nodular shadows and patch shadow. (4) Diffuse type (1 case): characterized by the nodules, most located on subpleural. Conclusion: Pulmonary cryptococcosis CT manifestations were manifold. Lesions were distributed in lower lungs of both sides and periphery. Multiple nodules or masses were frequent. Halo sign, hole, ground-glass shadow, air bronchogram were some significant. Diagnosis was dependent on the etiology and pathologic examination. |
