| 目的： 通过研究寻常性痤疮患者Th17细胞的相关指标，观察加味五味消毒饮治疗中重度痤疮疗效以及对Th17细胞的影响。方法：健康对照组30例，轻度痤疮组32例以及中重度痤疮组45例，检测三组外周血IL-17水平和Th17细胞在外周血中占CD4 细胞比例。45例中重度痤疮患者随机分成两组，治疗组采用加味五味消毒饮治疗（n=25），对照组采用异维A酸治疗(n=20)，比较临床疗效和治疗前后Th17细胞相关指标。结果：健康对照组、轻度痤疮组两项指标显著低于中重度痤疮组（P<0.05），健康对照组和轻度痤疮组之间无显著差别(P>0.05)。治疗组和对照组治疗前后，外周血Th17细胞相关指标显著降低（P<0.05）。结论：加味五味消毒饮和异维A酸都能有效的治疗中重度痤疮，治疗后外周血IL-17水平和Th17细胞在外周血中占CD4 细胞比例显著降低。 |

| Objective： To evaluate the potential association of Thl7 cells with acne vulgaris,and to observe the clinical therapeutic effect of Jiawei Wuweixiaodu Decoction(JWD) for treating the patients with midrange and severe acne vulgaris(MSAV). Methods： three groups, 30 healthy donors, 32 patients with mild acne vulgaris 45 patients with MSAV. In all three groups, The cytokines IL-17 in the peripheral blood by enzyme-linked immunosorbent assay(ELISA). the proportion of Th17 cells of each group were evaluated by flow cytometry．The 45 patients with MSAV were divided into two groups by random: treatment group (n=25) and control group (n=20), they were treated with JWD and isotretinoin respectively, The cytokines IL-17 in the peripheral blood and the proportion of Thl7 cells of each group as well as the clinical therapeutic effect of the treatments were observed and compared. Results： The two items about Th17 cells, healthy donors and patients with mild acne vulgaris were significantly lower than the patients with MSAV(P<0.05), but, there was no significant difference between healthy donors and patients with mild acne vulgaris(P>0.05).The total effective rate in the treated group and the control group was 84% and 85% respectively, comparison between them showed insignificant difference. The two items about Th17 cells lowered significantly after treatment in both treated group and control group. (P<0.05), there were lesser side-effect in treated group. Conclusion：Th17 cells might be correlated with the development of MSAV and the formation of inflammatory lesion. JWD and isotretinoin has definite clinical effect in treating MSAV, and lower the cytokines IL-17 in the peripheral blood and the proportion of Thl7 cells. |
