| 目的 分析脐尿管癌的多排螺旋CT(MSCT)表现及临床特点,探讨MSCT对其的诊断价值。方法 回顾性分析13例脐尿管癌MDCT表现及临床特征。结果 13例肿瘤呈类圆形结节4例,不规则团块形9例,主体均位于脐尿管走行区,最大直径约3-19cm。10例侵及膀胱顶壁或前壁并不同程度侵入膀胱内, 3例位于脐尿管上、中端。10例肿瘤边界相对较清晰,3例侵犯周边肠管及网膜结构。 CT平扫均呈不均匀的较低密度,4例病灶内见斑点状或不规则钙化；增强扫描呈持续性不均匀较明显强化,4例合并腹、盆腔淋巴结转移,所有病例周围肠系膜及脂肪间隙较清晰,未见异常密度影。结论 MSCT有利于显示脐尿管癌病理特征,对其诊断及鉴别有较大价值。 |

| Objective To analyse the manifestation of MSCT and clinical character for urachal carcinoma, and explore the diagnostic value of MSCT. Methods The MSCT findings of 13 urachal carcinomas proved by pathology were analysed retrospectively．Results Among 13 cases 4 cases showed oval nodes and 9 cases showed irregular masses, the main part located in the midline of the Retzius space along the course of the urachus ,the maximum diameter were nearly 3-19cm; and 10 cases in the top or front wall of bladder and intrude into bladder, 3 cases in the superior end or center of urachus, 10 cases showed clear boundary and 3 cases intrude the adjacent small intestine or mesentery. On unenhanced CT all tumors showed soft tumor with asymmetrical low density, irregular or spotted calcification can be seen in 4 cases; On enhanced CT, 13 tumors showed asymmetrical continuous evident enhancement, 4 cases accompanied with the metastases of lymphatic nodes, the clearance of mesentery and epiploon of all cases were clear, abnormal density can not be seen.Conclusion MSCT can show the pathological characters of urachal carcinoma clear and is important for the dianosis and identification for urachal carcinoma. |
