| 目的：观察无活性内皮细胞的冷冻保存角膜和使用Optisol中期保存液的新鲜角膜应用于直径小于3mm角膜组织缺损的周边角膜穿孔伤病人，行急诊治疗性角膜移植术后效果的比较。方法：回顾性分析2008年3月至2012年6月，因各种原因导致的周边角膜穿孔，在本院急诊行治疗性角膜移植术的患者10例（10眼），角膜穿孔范围均小于3mm，其中使用冷冻保存角膜6例（6眼），使用新鲜角膜4例（4眼）。术后均随访6个月以上。结果：所有患者均获得临床治愈，眼球完整性得以保持，术后前房形成良好，其中冷冻保存角膜组5眼及新鲜角膜组3眼术后随访6个月角膜保持透明，冷冻角膜及新鲜角膜组各有1眼发生术后混浊，新生血管长入。结论：无活性内皮细胞的冷冻保存角膜和新鲜角膜均可用于角膜穿孔伤患者的急诊治疗性角膜移植术，术后效果相近，可不行二次光学角膜移植手术，保持角膜的透明。冷冻保存角膜满足了急诊手术的需要，也缓解了新鲜角膜紧缺的现状。 |

| Objective: To observe the application of frozen preserved cornea without active endothelial cells and fresh cornea preserved with Optisol storage medium in patients with corneal perforation and tissue defect below 3mm. And compare their effects after emergency theraputic keratoplasty . Method: A retrospective survey study was conducted on 10 patients (10 eyes) with emergency theraputic keratoplasty in our hospital from March 2008 to June 2012, who had peripheral corneal perforations with tissue defect belowed 3mm caused by various reasons. In these 10 patients, 6 cases (6 eyes) used frozen preserved cornea and 4 cases (4 eyes) used fresh cornea. Post-operation follow-up lasted for over 6 months. Results: All patients obtained clinical cure. The integrity of eyeball was maintained. After operation, anterior chamber formed well. 8 corneas kept transparent. One eye respectively in frozen cornea and fresh cornea groups had corneal opacity and neovascularization. Conclusions: Both frozen preserved cornea without active endothelial cells and fresh cornea can be used in emergency theraputic keratoplasty of corneal perforation. They have similar postoperative effects and maintain transparency without secondary optical keratoplasty. Cornea with frozen preservation can meet the requirement of emergency operation and alleviate the situation of shortage of fresh cornea. |
