| 目的：探讨双管喉罩用于老年患者结直肠手术全麻中的有效性和安全性。方法：46例行择期手术的老年患者,术前情况评估ASA Ⅱ～Ⅲ级,随机分为双管喉罩组(A组)和气管导管组(B组),每组23例。两组麻醉诱导、维持用药及麻醉机设置参数相同。记录诱导前、插管(喉罩置入)前1分钟、插管(喉罩置入)时、插管(喉罩置入)后3分钟及拔管(拔喉罩)前、后1分钟、拔管(拔喉罩)后3分钟的MAP、HR等指标,术毕有无拔管呛咳、拔管体动、咽喉疼痛、声音嘶哑、咳嗽咳痰等不良反应,并记录手术时间、苏醒时间。结果：苏醒时间A组快于B组(p<0.05),A组在T2～T6时MAP明显低于B组,HR慢于B组(P＜0.05),A组拔管呛咳、拔管体动、咽喉疼痛、声音嘶哑、咳嗽咳痰等不良反应明显少于B组(p<0.05)。结论 双管喉罩用于老年患者结直肠手术全麻管理,血流动力学稳定,苏醒迅速,不良反应少。 |

| Objective: To explore the efficacy and safety of general anesthesia for colorectal surgery with the proseal laryngeal mask airway in elderly patients. Methods: 46 elderly patients with colorectal disease who were operated by selective surgery were equally divided into two groups randomly, general anesthesia of group A was performed by using proseal laryngeal mask airway and group B by endotracheal tube. All patient ASA grade was included inⅡ~Ⅲ. Anesthetic induction and maintenance medication and parameters of anesthesia machine were the same in two groups. The index of mean arterial pressure(MAP) and heart rate(HR) was recorded at below time points: before induction, 1 minute before intubation (or PLMA), intubation (or PLMA), 3 minutes after intubation (or PLMA), 1 minutes before and after extubation (or PLMA) and 3 minutes after extubation (or PLMA). The adverse reactions of extubation bucking and body movement and sore throat and hoarseness and cough sputum were recorded, the operation and recovery time were recorded simultaneously. Results: The recovery time in group A was faster than in group B (p<0.05). MAP in group A was significantly lower than in group B and HR in group A slower than in group B in time point at T2 ~ T6 (p<0.05). The adverse reactions of extubation bucking and body movement and sore throat and hoarseness and cough sputum in group A were significantly less than in group B (p<0.05). Conclusions: General anesthesia for elderly patients with colorectal surgery by using proseal laryngeal mask airway can lead to hemodynamic stability and rapid awakening and fewer adverse reactions. |
